Housing Exemption Form This form is used for incoming students who are exempt from housing because they will be living with a parent or guardian within a 50 mile radius.
Housing Cancellation Request Form This form is used for students who are wishing to cancel their housing either mid-semester or between Fall and Spring Semester. Please note that this form is only a request and does not guarantee cancellation. A cancellation fee will also apply in most cases (excluding December graduates).
Overnight Guest Form This form is used to students wanting to have an overnight guest. This form must be completed, signed, and returned 24 hours before your guest is due to arrive. Please view all overnight guest guidelines here before turning in form.
Break Stay Form This form is completed if students will be staying on Campus during the Winter Break. The form must now be completed in the student's Self Service account. The Winter Break form will be available beginning December 1st. Please note that fees may apply.
